Appointments
To schedule an appointment please call 847-674-1200.
Cancellations
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hour notice. This courtesy, on your part, will make it possible to give your appointment to another patient. Failure to do so will result in fee of $30.00.
Third Party Liability
For patients treated for injuries from an automobile or work injury, we will attempt to receive authorization from the appropriate carriers before your visit. If prior authorization is not received, you will be responsible for the visit.
Updating Records
It is very important that your doctor have your current address, work and home phone numbers so we are able to contact you, with an additional emergency contact number if possible. Please notify the receptionist of any changes.

Prescription Refills (Monday - Friday, 9am - 5pm)
Please contact your pharmacy and they will send us electronically or by fax your request for a refill. Allow up to 24 hours to process.
FILLING OUT FORMS FOR ADMINISTRATIVE PURPOSES
If the Doctor has to fill out forms regarding your care for administrative purposes, you may be charged $25. These forms should be returned to you within 10 business days.
ACCESS TO ELECTRONIC HEALTH RECORD
Our goal is to provide you with your personal health record which gives you 24/7 online access. Your personal health record contains your medical history including diagnosis, medications, immunizations, appointments, allergies and in the future laboratory test results. By providing us with your email address your doctor will grant you access to Patient Fusion, the platform used to accomplish this. You will receive through your email an 8 capital letter character that you will use to set your own login and password. Also you will get reminders of your appointments one week and one day before. Be aware that Nora Medical Group does not communicate by email with our patient’s or discuss labs by this medium. If you are not a health professional, any questions pertaining to your health record including labs should be discussed with your physician by making an appointment by calling the office at (847) 674-1200 option 0.
