Appointments
To schedule an appointment please call 847-674-1200.
Cancellations
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hour notice. This courtesy, on your part, will make it possible to give your appointment to another patient. Failure to do so will result in fee of $25.00.
Third Party Liability
For patients treated for injuries from an automobile or work injury, we will attempt to receive authorization from the appropriate carriers before your visit. If prior authorization is not received, you will be responsible for the visit.
Updating Records
It is very important that your doctor have your current address, work and home phone numbers so we are able to contact you, with an additional emergency contact number if possible. Please notify the receptionist of any changes.

Prescription Refills (Monday - Friday, 9am - 5pm)
Please contact your pharmacy and they will fax to us your request for a refill. Allow up to 24 hours to process.
