New Patients
If you are a new patient, please bring to your office visit the following items:
- Your insurance card and copayment amount
- Credit or debit card
- All your medications
- All your previous medical records or the Nora Medical Group authorization for use and disclosure of medical information form completed (found below)
- The Nora Medical Group patient survey completed (found below)
- E-mail account and pharmacy information
