New Patients
If you are a new patient, please bring to your office visit the following items:
- Your insurance card and copayment amount (your copayment is due at the time of your visit. If this is not payable, your appointment will be rescheduled)
- Credit or debit card
- All your medications
- Please submit previous medical records prior to your visit. You will not be seen by the physician until these medical records are reviewed.
- Nora Medical Group authorization for use and disclosure of medical information form completed (found below)
- The Nora Medical Group patient survey completed (found below)
- E-mail account and pharmacy information
